Hartland Community Nurses
The Community Nurses are available for home visits to older Hartland residents for assessment, support and referral to appropriate resources so residents can age in place, at home or at a residence of their choice.
The Nurses can be a helpful point of contact for families who live far from Hartland, assuring them when all is well and alerting them to issues or conditions that need attention. Home visits are also a continuing support to elders returning to their Hartland homes after a hospitalization or rehab. The Community Nurses do not provide direct medical care.
One of a growing number of Community Nurses programs in the Upper Valley, our nurses are supported by the Town of Hartland, Upper Valley Community Nursing Program, the Ottauquechee Health Foundation and private donations. Aging and Able
Our Aging in Hartland community nurses are reaching out to Hartland residents aged 80+ in hopes to connect and provide support to age well in place by improving health and safety in the home. The Aging and Able program is a free support service, connecting Hartland senior residents with nurses to assess safety, falls risks, food security, and the overall ability to age in place. Being proactive in planning for services such as transportation, home health care, meals-on-wheels, identifying a support system, as well as small home updates such as grab bars, can greatly increase the ability for people to stay safely in their homes for longer.
We are able to help connect community members to resources in the area as well as make home assessments to reduce fall risks and promote independence in the home. Having a visit with one of the nurses and assessing these risks and needs before a fall or issue arises can help residents age well in place.
Community nurse: Nancy North
Nancy North started as a community nurse for Hartland in June 2021. She is a retired Dartmouth Hitchcock nurse who decided to explore parish nursing, a specific sort of community nursing. Community Nurse: Rebecca Maxham
Rebecca is a local Hartland resident, who is dedicated to caring for those in her
community. After graduating in 2019 from Vermont State University with her RN,
Rebecca has been working primarily in medical-surgical nursing, giving her experience
with diverse patient populations and a variety of acute and chronic conditions. Rebecca
has a special appreciation for the elderly population and feels passionate about
